Students may request a copy of their Co-Curricular Transcript through the Office of Student Life and Leadership or at cocurriculartranscript@uaptc.edu.
If a student wishes to have a co-curricular experience added to their co-curricular transcript, please submit the Student Co-Curricular Transcript Activity Submission Form.
